What is the health declaration?

You and all of your included family must read the Health Declaration and answer “YES” or “NO” to the health question on the Ajuria questionnaire

Health Declaration

By answering “NO” you confirm that you and your family DO NOT intend to:

  • enter a hospital or a healthcare facility (including nursing homes) while in Australia;
  • work as, or study to be, or train to be, a doctor, dentist, nurse or paramedic during your stay in Australia;
  • work, or be a trainee, at a child care centre (including preschools and crèches) while in Australia;
  • be in a classroom situation for more than 3 months (e.g. as either a student, teacher, lecturer, or observer)
    while in Australia.

 

And that you have NOT:

  • and have never had tuberculosis;
  • been in close contact with a family member that has active tuberculosis;
  • been in contact with anyone (not just family members) who is known to have TB;
  • ever had a chest x-ray which showed an abnormality.

 

And that you DO NOT expect to incur medical costs, or require treatment or medical follow up for:

  • blood disorder*;
  • cancer*;
  • heart disease*;
  • hepatitis B or C and/or liver disease*;
  • HIV infection, including AIDS*;
  • kidney disease, including dialysis*;
  • mental illness*;
  • pregnancy*;
  • respiratory disease that has required hospital admission or oxygen therapy*;
  • other*.

*If you answer yes to any of the points marked with an asterisk above, we will need details of the medical condition (not the costs likely to be incurred) to complete the visa form.

 

And that you:

  • have not ever been admitted to hospital and/or received medical treatment for an extended period for any reason (including for a major operation or treatment of a psychiatric illness);
  • do not have and have never suffered from mental health problems;
  • have not ever been told you are HIV positive;
  • do not have and never had hepatitis or problems with your liver or yellowing of the skin
  • have not had cancer in the last 5 years
  • do not have and never had high blood sugar/diabetes
  • do not have and never have had heart problems, including high blood pressure or a heart condition that you were born with;
  • do not have any blood conditions
  • are not pregnant
  • do not have and never have had bladder or kidney problems
  • do not have a physical or intellectual disability that makes it difficult for you to function independently (for example, to move around or learn) or work full-time
  • do not take drugs or drink alcohol regularly
  • are not taking any prescribed pills or medication (excluding oral contraceptives, over-the-counter medication and natural supplements)?

 

If any of the above does apply then you must answer “YES” on the Ajuria questionnaire and provide details.

Will my family need to do the same?

Yes.

How do I arrange a medical if I need one?

  • If you are in Australia:
    Health checks must be completed with BUPA, who are the Department of Home Affair’s migration medical service provider. Please do not book an appointment until we provide you with your referral letter.
  • If you are outside of Australia:
    Health checks must be completed at one of the clinics approved by the Department of Home Affairs.  Contact details for the clinic closest to your location can be found here. Please do not book an appointment until we provide you with your referral letter.

What happens then?

  • The results will be sent directly to the Department
  • We can check online to see when they have been received and processed.

How long are the medicals valid for?

12 months.

What if I fail?

A health waiver may be available. We will advise you further on this if necessary.
